2010- 2011 INFORMATION SHEET
September 7, 2010 - Aug 1, 2011

We are excited to have Chelle Jennings-Seyfert and The Dance
Academy dancers joining us this year!

REGISTRATION FEE:
$10.00 single student, $15.00 family - covers
studio year Sept 2010 to Aug 2011

MONTHLY FEES - Due the first class session of every month. If a
family is enrolling more than one child, the first child's instruction is full
price. Additional siblings receive a $5/mo. discount. Termination of
services by the parent/guardian must be done in writing and dated, in
order to avoid future billing. All payments may be brought to the studio
during business hours, mailed in, or deposited in our secure locking
mailbox. Payments may be made by Credit card, check, or money
order.   Cash payments should be made to our desk personnel so they
can write out a receipt

The fee is the same for all months September through May.   There is
neither reduction for “short months”, nor an increase for months with 5
lessons.

30 minute class $30.00, 45 minute class $34.00, 60 minute class
$38.00, 90 minute class $46.00, 2 hr class $54.00, each additional 30
minute class time $8.00  Maximum family fee $120.00 per month.

LATE FEE - There is a $15 late fee per month for lessons paid after the
15th of the month ,  and for late costume fees. This is non-negotiable.
Unpaid fees may result in student not being allowed into class,
expulsion from the recital, the inability to sign up for future classes, and
your debt being turned over to a collections agency.
Any dancer more than 2 months in arrears will not be allowed to
dance.  If you have a financial problem, talk to me or our office staff,
we may be able to make special arrangements.

DROP-OUT FEE - Students withdrawing from Vivette’s Dance Studio
the 2nd semester (Jan. 1st- Recital) create problems for the other
dancers as well as the teachers, as choreography must be changed to
accommodate their absence. Therefore, a Drop-Out fee equal to a
month's tuition for the number of classes dropped will be assessed to
your account.

WITHDRAWAL FROM CLASSES: You must give me written notice of
your dancer’s withdrawal from classes 2 weeks prior to the beginning of
the month.  A withdrawal from studio form is available. Not showing up
anymore does not count.  If you do not notify me in writing, of your
withdrawal, you will continue to be billed for classes for up to two
months at which time we will remove you from our enrollment and turn
the unpaid account over to collection..

RECITAL FEE - $25 single student/ $30.00 family  is due by October
30th for those who want to participate (non-refundable after Jan. 1st).
(2nd Semester students in Feb). Costume Deposits for Recital costumes
are due by Dec. 15th.  (2nd Semester Students in Feb) Costumes are
ordered before Dec 31st, after which parents are held liable for their
child's costume expenses. If you do NOT intend to get costumes, you
MUST inform the studio in writing, before Christmas Break, or pay a
$15 per costume cancel fee.

RETURNED CHECK FEE - There will be a $30 charge on all returned
checks.

ABSENTEES, TARDIES: Roll will be taken at each lesson.  Please
assist your child in having regular attendance so that they will not fall
behind the rest of the class.   Your absence does not reduce our cost of
doing business; there are no adjustments for missed lessons.

RECITAL INFORMATION:

“?? ”
will be our 36th annual recital.
The Recitals will be Saturday, May 21 1:00 and 4:00pm.

There is no admission charge for the recital.

COSTUME INFORMATION:
Each student will be required to purchase a costume (s) for the recital.
The following prices may change due to the world situation (sequins are
a petroleum product!)

A deposit towards the actual cost of the costume of $50 for Bitty
through Primary 1, $80 for Primary 2, $120 for Primary 3 will be posted
to your account in November. Jr and Senior Deposits will be $60.00 per
costume.
Costumes for Pom, Adult and students taking  single subject classes will
be billed on an individual basis.
In general, child sized costumes are approximately $50, adults $60.

The costume deposit is due by Dec 18, 2010
You may pay the deposit in full or in payments.
A $15.00 per costume late fee will be assessed after that date
Voluntary fundraisers to help parents pay for dancers costumes may be
held.

Occasionally a class will need to order a pair of shoes in a different
color, especially if the class will be attending a competition where leg
color must match shoe color.  We try to avoid this, but it is sometimes
necessary.

Deposit payments are refundable until I order the costumes.  After all
costumes and accessories are ordered you will either be billed for the
remainder due or receive a credit on your account l for the difference
between the deposit and the actual cost.  We generally do not go over
the deposit amount; we try very hard to keep costume costs
reasonable, reusing and recycling when possible.

DRESS CODE:
All dancers are expected to abide by the dress code..
Non-compliance may result in a student not being admitted to class.
The Dress Code was implemented for the dancers safety and
education.
You wouldn’t wear a swimsuit to basket ball practice,
you don’t wear play clothes to dance class.

Hair:
Bun (preferred)  braid or ponytail.  Loose hair is a safety hazard
and a distraction to the dancer.  A ponytail whipped across the eyes by
centrifugal force during a turn is painful and dangerous.

OTHER PERFORMANCES:
We perform at many nursing homes, clubs,
malls, and basket ball games through out the year.  These
performances are very important to the dancer's growth as a performer
and also to the audience.  I expect the dancers to attend as many of
these performances as possible.

Younger dancers may perform their Christmas dances at several
nursing homes in Dec. and early January.
We will try to perform at several nursing homes in May.

COMPETITION: We hope to attend regional competitions possibly in
Kansas City, Oklahoma City, and Wichita   Any dancer with the desire to
compete may do so as a member of our StarFeet Academy Competition
Team.  We have 4 teams, Mini Stars Pre-K, Rising Stars grades 3rd-6th
grades, Shining Stars  Middle school and Shooting Stars for High School.

Our STAR FEET ACADEMY“ Competition dancers perform each month at
area nursing homes and other exhibitions.

DANCE MASTERS: Miss Vivette is a member of Dance Masters
America, Heart of America Chapter out of Kansas City.  Members of
DMA are “certified by test to teach”, meaning that applicants must pass
rigorous tests in at least 2 subjects and submit proof of continuing
education hours each year.

SALINA DANCE ASSOCIATION: Vivette’s Dance Studio is a member
of the Salina Dance Association.  We abide by all SDA policies.

OBSERVATION WINDOWS: Parents are able to observe classes
through our observation windows.  The blinds will always be “open”
unless there is a need for privacy such as measuring for or trying on
costumes.

T-SHIRTS ,JACKETS,CAR EMBLEMS: Recital T-shirts are passed
out to each student in March.  All students receive a recital T-
shirt.   Studio and Star Feet Jackets may be ordered several
times during the year.   We may take orders for or sell other T-
shirts during the year.  Car Decals may be ordered at the
Dancewear sale and at other times during the year.

PARKING:
Please be considerate of the homeowners in the
neighborhood when parking.  Parking is available along the street and
unless it is “Guard Saturday”, in the Armory’s lot.

NO STREET SHOES IN THE STUDIOS PLEASE!   They bring in dirt
and sand, which is very unpleasant for the dancers, when they work on
the floor.  We have a locker-room in our studio if needed to change
from or into street clothes.   Teachers and Sr Dancers may “claim” a
locker.  Street shoes and coats should not be brought into the studios.

ACHIEVEMENT AWARDS: All students who complete the year of study
will receive an achievement award.  These achievement awards are a
gift from me to my students as recognition of their hard work this year.
Awards are picked up at the after recital parties.

NOTICE: Vivette’s Dance Studio takes photos and video’s of our
dancers in the studio and at performances.  Vivette’s Dance Studio has
the right to use these images for publicity and commercial purposes on
our website, newspaper ads and TV commercials.  No compensation will
be given for the use of these images.  If you do not want us to use your
dancers image you must notify us in writing.
